A plain-English guide

Costs and insurance

How much will the complete plan cost?

Request the full bill that applies once the opening offer is over.

Am I comparing the same kind of price?

The price of a brand-name weight medicine can vary with the way you pay.

A drug company publishes a list price before any discounts.

If you pay without insurance, a cash offer sets the price.

With insurance, your share of the bill depends on what your plan pays.

An online plan may charge a membership fee and leave medicine as a separate cost.

Ask each service to separate the visit, tests, medicine and later care in writing.

Cash offer: check later supplies

This source information was recorded on September 26, 2026.

For eligible new U.S. patients paying outside insurance, the semaglutide (Ozempic) injection offer covered the first two fills at $199 per monthly supply.

A fill is the medicine supplied each time a prescription is used.

The opening price applied only to eligible strengths and adults aged 18 or older.

Here, strength tells you how much medicine is in one prescribed dose.

Later monthly supplies were priced at $349 or $499, depending on strength.

These offer terms run through December 31, 2026.

An insured person using this cash offer cannot ask the insurer to pay them back or count the payment toward their insurance deductible.

A deductible is what a plan requires you to pay before it starts paying covered costs.

Medicare: check who qualifies

This source information was recorded on September 26, 2026.

Under a temporary program through Medicare’s drug plans, qualifying people could get certain weight medicines for $50 per monthly supply.

The program starts on July 1, 2026, and ends on December 31, 2027.

To qualify, a person must be 18 or older and enrolled in a Medicare drug plan accepted by this program. Their weight and health conditions must fit the program’s rules. Payment requires both a prescription and approval from the program.

Type 2 diabetes makes a person ineligible for this temporary program, for example.

Ask your prescriber to check your health history and drug plan against the full rules before quoting this price.

The $50 payment is handled separately from your usual Medicare drug plan. It does not count toward that plan’s annual cap on payments for covered medicines.

You can still owe the $50 even after reaching that limit for your other covered medicines.

Coupons cannot reduce the $50 payment.

Will insurance pay?

Medicare’s usual drug plan may cover certain medical uses besides weight loss. Treating diabetes and reducing certain heart risks are examples.

The exact medicine and reason for taking it still have to meet the plan’s medicine-list and approval rules.

Drug companies offer savings cards, like coupons, to lower some patients’ share of a medicine bill.

The cards for the main brand-name weight medicines require private or workplace insurance. People with Medicare or Medicaid cannot use them.

A drug company’s cash offer has different rules. Someone with Medicare or Medicaid may still qualify by paying the whole cash price without using insurance.

Ask your insurer to check the named product and why it was prescribed, then explain your part of the bill.
